Obas Lab Diagnostics & Wellness does not accept insurance for services rendered unless explicitly stated. We are not reimbursed by Medicare, Medicaid, managed care plans, or other third-party payors for wellness services, IV hydration, supplements, vitamins, or pharmaceuticals.
Clients are financially responsible for all services provided, regardless of whether reimbursement is sought or received from an insurance carrier or third party.
Payment is due at the time of scheduling or at the time of service, as indicated. By scheduling an appointment, you authorize Obas Lab Diagnostics & Wellness to charge your selected payment method for all applicable fees and taxes.
Clients are responsible for ensuring that payment methods remain valid and sufficient.
If we are unable to provide services due to availability or unforeseen circumstances, a credit or refund will be issued. Refunds or credits are not provided due to dissatisfaction with services
If a medical professional is unable to complete the IV portion of a scheduled service after an attempted IV insertion due to circumstances beyond their control, a credit or refund will be issued minus a $100 medical assessment fee.
Clients must provide accurate and complete personal and medical information. If inaccurate information is provided, if a client misrepresents their age or medical history, or if a client appears under the influence of drugs or alcohol at the time of service, the full-service fee will be charged, and services may be refused.
Services provided by Obas Lab Diagnostics & Wellness are not a substitute for primary care or emergency medical treatment. No guarantees are made regarding outcomes. Clients should seek emergency medical care when appropriate.
Telehealth services are provided in accordance with applicable state and federal regulations. Availability of telehealth services may vary based on location and clinical appropriateness.
